The mattress and other accessories are all optional as you can see listed on the website. They are sold separately as optional accessories.
Does this bed come fully assembled?
Yes, the bed frame comes fully assembled. However, you need to install the head & foot boards and bed rails of your choice since they are available as optional accessories. The installation is very easy.
What does bariatric mean? On this model is the entire bed raised by a motor or od just the head and feet area raised by a motor. Which of all of your bed models raises the entire bed up the highest?
Bariatric represents people of obesity. Bariatric products need to be bigger in size and higher in weight capacity. Bariatric hospital beds can usually hold more than 450 lbs and are wider than standard size 36". This model AF4005 is semi electric hospital bed that can raise the head and foot sections by a motor controlled electrically. The height of the bed level is adjusted manually with a crank.

If you need to have the bed height adjusted electrically too, you need to select a full electric hospital bed. The Zenith full electric hospital bed offers the widest range of bed height on the market from 7.9" to 30". It is more used in care giving facilities such as assisted living or nursing home. A typical hospital is adjustable with the bed height and the head / foot sections can be also raised up or lowered down.

For a semi-electric hospital bed, the head / foot sections can be raised up and lowered down electrically controlled by pushing buttons on a hand controller. The actions are driven by a electric motor. However, the bed height has to be adjusted manually with a crank.

For a full-electric hospital bed, all of the above actions, adjustments of bed height or head / foot position, can be controlled electrically with a hand controller.
