You can not put a regular queen size mattress on it. The mattress will be damaged. You need a hospital mattress specifically used for hospital bed. You can select the package that comes with the bed frame, mattress and rails. The rails are virtually given to you free of charge in the package. If you purchase the bed and mattress separately, you will pay more than the package price. You can select trapeze of your choice on the following page:
