As described on the website, the Maxi-comfort model has two motors that control the back and footrest moving independently. In contrary, the 3-position model has only one motor that controls both back the footrest moving proportionally. This means that when the back reclines, the footrest move up flat, while when the back raises up, the footrest move down, and vice versa.
